Kroger Members: Get Your Health Screening and Save $$$

Every year Kroger UFCW 227 members enrolled in health insurance through the Heartland Health & Welfare Fund have completed a biometric health screening. Completing your screening is your opportunity to know your numbers and save money every week for completing it!

By completing and returning your wellness passport, you can save up to 30 percent on your weekly premium for medical benefits in 2023 – you don’t want to miss out on those savings. There is no requirement that you “pass” any numbers. All you have to do is compete the screening, turn it in to the Heartland and save money every week.

The biometric screening checks your BMI, blood pressure, cholesterol and blood glucose. Make an appointment with your primary care doctor, at The Little Clinic or at a Kroger Pharmacy.

Remember: it’s important to fast 12 hours in advance of the screening! You’ll also want to bring your wellness passport and medical ID card to your appointment.

It is your responsibility to mail, email or fax your completed wellness passport to the Heartland Health & Wellness Fund! You will not receive credit for your screening unless you return the form, completed and signed, to the fund.
