Kroger Union Members: Health Screening Deadline is Extended to October 15th

As Americans resume normal activities, many are using this opportunity to schedule necessary medical appointments they may have been unable to complete during the pandemic.

Annually, Kroger Local 227 Union participants in our Health & Welfare Fund have completed a biometric health screening. The program looks a little different this year under the Heartland Health & Wellness Fund, but the important part remains the same: the screening is your opportunity to know your numbers and earn an incentive for completing it! By completing and returning your wellness passport, you can save up to 30 percent on your weekly premium for medical benefits in 2022 – you don’t want to miss out on those savings.

The biometric screening checks your BMI, blood pressure, cholesterol and blood glucose. Make an appointment for a screening at Kroger Pharmacy (by calling 877-444-9689), with your in-network primary care physician or at The Little Clinic.

Remember: it’s important to fast 12 hours in advance of the screening! You’ll also want to bring your wellness passport and medical ID card to your appointment. To download and print a copy of the Wellness Passport click here.

It is your responsibility to mail, email or fax your completed wellness passport to the Heartland Health & Wellness Fund! You will not receive credit for your screening unless you return the form, completed and signed, to the fund.
