2026 Kroger Health Screenings: What You Need to Know

It’s time to complete your 2026 health screening an important step to protect your health and keep your healthcare costs down.

By completing your screening, you can qualify for the “with health screening” rate on your 2027 medical premiums helping you save money while staying on top of your health.

You don’t have to “pass” any tests. All you have to do is complete your screeing by Septmber 15th to qualify for a lower price on your weekly insurance premiums.

Deadline

All screenings must be completed and submitted by September 15, 2026.

What You Need to Do

1. Schedule Your Screening

You can complete your screening:

• With your in network primary care provider
• At a Kroger Pharmacy
• At a Kroger Little Clinic

Screenings are available now through September 15, 2026

2. Bring the Right Materials

At your appointment, make sure to bring:

• Your medical ID card
• Your health screening form

Your provider will record your results and sign the form.

3. Complete Required Tests

Your screening will include:

• Blood pressure
• BMI
• Blood glucose
• Cholesterol HDL LDL total
• Triglycerides

Important- you must fast for 12 hours before your screening.

4. Submit Your Form

Once completed, it is your responsibility to return the form to the Fund. Make sure to snap a picture or keep a copy for yourself in case it gets lost in the mail or at Heartland Health Fund’s office.
