How UFCW 227 Members can file a Lincoln Financial Claim
Some UFCW 227 members have union contracts that allow you to enroll in short term disability, life insurance, critical illness or other types of elective coverage and pay directly from a payroll withdrawal. This is a great benefit for union members since signing up on your own doesn’t always have the same purchasing power as our strength in numbers does, and since it’s a payroll withdrawal you have some security in the fact that a missed payment, changing bank cards, or insufficient funds won't mean you lose your coverage.

Another perk to UFCW 227’s Lincoln Financial Benefits is that Insurance Benefits for Organized Workers (IBOW) administers these benefits for us. That means when you need information or to file a claim you can call a small local office. They are able to make sure you file a claim on all policies that apply to your situation, help you to gather all of the information you need and are experienced in filing complex claims to make the whole claims process easier.
Steps to File a Claim
1. Call IBOW to get claim forms at 502-473-1300.
2. Take the physician's form to your treating doctor.
3. While at the doctors office make arrangements to get your medical records from the date you were first treated for your current condition.
4. Fill out the employee form with your personal condition. Don't forget bank information if you want an automatic deposit.
5. The employee form can be completed by your HR person or send it in blank and IBOW can get this form filled out.
6. Fax all forms to IBOW at 502-473-0790.
7. You may not get the records right away so send in the forms when you have them filled out and follow with the medical records when you receive them.
