Heartland Health Dependent Verification
Heartland Health Fund is auditing who is covered on your insurance. Kroger, Dawn Foods, Prairie Farms Dairy, Jackson House and Banks Grocery get their insurance coverage from Heartland and if you cover anyone except yourself you may have received a Dependent Verification notice on submitting paperwork to keep their coverage.
How you are Notified
The Heartland Health Fund is working with a independent company called Secova on the audit, all paperwork goes to Secova and all information about your specific situation will need to go through Secova as well.
Secova has sent text messages, emails, phone calls, and letters to members who need to send paperwork to them for the audit. Below are examples of an email Secova sent as well as a text message that was sent.
What Documents you Need to Send
Depending on who else is covered on your insurance they could need things you don’t expect, for example if you cover your spouse you may think a Marriage Certificate is enough but Secova may also be looking for a mortgage statement or lease statement or bank statement with both your name and your spouses names listed. How to Send Your Paperwork to Secova
Secova gives the following options for submitting paperwork. If you choose to upload, mail, or fax your paperwork make sure you keep a copy in case you need to send it again, if you email your paperwork don’t delete the sent email in case it needs to be resent.

Annual Health Screening
To make sure you have the insurance coverage you need at the very best price, be on the lookout for your Annual Health Screening. These screenings can help save you hundreds of dollars a year!
Remember that you don’t need to “pass” the screening metrics, you only need to have it filled out by a healthcare provider, and sent to the Heartland Health Fund following the instructions on the form. Please keep a copy of the screening for your records and to fix any errors that may happen after you submit the form.
Open Enrollment
Keep an eye out for Open Enrollment, this is your time to join the insurance coverage, add spouse/dependents, or change coverage.
