Kroger Members: Get Your Health Screening Done Early

Every year Kroger UFCW 227 members enrolled in health insurance through the Heartland Health & Welfare Fund complete a biometric health screening. Completing your screening is your opportunity to know your numbers and save money every week on your insurance premiums! Health screenings must be completed by September 15th, but don’t delay, complete your screening today!

Get Your Screeing Done Early! 〰️

By completing and returning your wellness passport, you can save money on your weekly premium for medical benefits in 2025 – you don’t want to miss out on those savings. There is no requirement that you “pass” any numbers. All you have to do is compete the screening, turn it in to the Heartland Fund to save money every week.

The biometric screening checks your BMI, blood pressure, cholesterol and blood glucose. Make an appointment with your primary care doctor, at The Little Clinic or at a Kroger Pharmacy.

Remember: it’s important to fast 12 hours in advance of the screening! You’ll also want to bring your screening form and medical ID card to your appointment.

It is your responsibility to mail, email or fax your completed wellness passport to the Heartland Health & Wellness Fund! You will not receive credit for your screening unless you return the form, completed and signed, to the fund.
